The transitions team help young people transfer to our adult service within the social care framework. This may seem obvious but it is interesting how the rules, processes, rights et al change on the birthday when a child becomes an adult.

This small team are unique in that they see and experience both the adult social care and childrens services. They were able to share with me comparative information between the two services and offer improvements.

I enjoyed my time with this dedicated and knowledgable team. I will be encouraging the council to further learn from their unique perspective. I took away a list of action points as always.
